In what context? CoT is the stack of paperwork you have to supply to show a film is yours, and won't be challenged by someone in 5 years claiming they wrote the story, or that the camera operator suddenly starts trying to exert rights. Initially, it will be every contract you have with every actor, crew, location, etc and any you have around story rights, screenplay rights, etc. Each agent/distributor will ask for something slightly different, and it will inevitably cost money getting things notarised, but only at the point where you have a sale.

OK, so you need a script purchase agreement, I may have an old sample somewhere. The writer will need to clarify if it's an original story or if there are any story rights you need to get cleared too. PM me your email address and I'll share some old sample contracts with you. I wouldn't use them without a legal opinion, buy it saves starting from scratch.

Alternatively (and if it's a short, my preference), you can write a simple agreement in plain English. There's no money to speak of at stake in a short film, so you don't need the same level of rigour in your agreements. You won't need a full chain if title for a short, unless for some reason you manage to sell it for proper money (this is extremely rare)

I hope you're well.
Thank you for coming to my rescue ones again.
The script is someone else and I'm taking it on board to produce. I don't want to go to a lawyer to draw these docs, I would rather get a cheapest solution. I know sometime the papers we find on the web may not be appropriate and not cover one legally. I hated to find out if there is a safe place that can provide them securely.

It is a short but I would like to have a proper documentation cause there is some budget involved, and that comes from an investor that I have to answer to in case something goes wrong.
