Any way you please or are contracted to do! My only suggestion is to keep it short - I've seen a 7' student short film with an extra 5' of end rollers, when a TV programme will get all the rollers done in 30s.

I'm just doing this very thing for a small team/crew of a 90 min animation. Best advice I had is open with a Banner for the film company that "Presents" then title of film followed by a film by ????, then screenplay by ???? and in our case Music by ???? - fade into film.
After the movie as in our case: In Memory of: ???? followed by "Producers wish to thank" list + small number of crew & Executive Producers/Producers/Director names and maybe a website.
In all about 5 to 6 mins-before cinema is empty!
