On a Bell?

I've never worked on a large enough set to have required a bell before and I haven't found a good reference for a standard procedure.

Where exactly does it fit in the sequence of steps? Who needs to be ready before it's called/rung and whose job comes immediately afterwards? Is there a standard ring pattern (e.g. one ring on, two off, for example) or is it just arbitrary? If there's no bell, is there a standard shout? And most importantly... who actually rings it? I guess it's 1st AD to call for it, but who's pressing the button / switching on the red light? Best guess is someone in the sound dept, but that's all I got so far.

In one sense, trivial questions... in another, the sort of the thing you'd want to know to avoid looking like an amateur when stepping up to a larger scale shoot!
