Any suggestions for finding a release form for archive footage

I have got permission from a well-known company to use up to a minute of sound and vision from an episode of their programme published on YouTube for my disability-led short film. I have the permission in email form but would like to find a suitable agreement/release form for them to sign. Can anyone suggest where I might find a suitable document for this? As it is a finished product but stars one (well-known) person, I'm not sure if it is a talent release form or an archive footage form that I need - or something else?

A simple letter suffices.

All you need is something from them confirming you are allowed to use their material in the way you want to use it. Even an email trail is OK in theory, if it's clear what is meant.

They should have all rights, so no need to go to talent.

Don't over complicate it.

Quite right and as Marlom says.

Law is a lot more basic common sense than is often sold by lawyers.
