Hi Andrew, even when you have an experienced crew many safety issues are caused by being behind schedule and rushing to get a scene shot or people doing more than one job at a time and not being able to fully concentrate on their given task. These issues combined with freelancers not wanting to "rock the the boat" for fear of not being re-employed can culminate in a tragedy such as the one on Midnight Rider.

Very sad - and entirely avoidable. On every shoot, no matter how small, someone has to take responsibility for the safety of the cast and crew.

Yes - it's small wonder serious accidents don't happen more often - particularly on indie shoots many of which (even here on Shooting People) don't have insurance, let alone fire officers or first-aiders.
A simple bit of advice for filmmakers out there - you should always at bare minimum have an emergency first-aider on hand - the best way to ensure that is to take the course yourself :)
